Shockwave Therapy for Erectile Dysfunction: A Non-Invasive Path to Better Sexual Health
If you're experiencing erectile dysfunction, you're not alone—and you don't have to just accept it.
Research estimates that between 30 and 50 million men in the United States are affected by ED. It becomes more common with age, but it is not simply an inevitable part of getting older. And yet, many men either don't seek treatment or don't know that options beyond medication exist.
One that's gaining traction—and for good reason—is shockwave therapy.
What Is Shockwave Therapy?
Shockwave therapy (also called Extracorporeal Shock Wave Therapy, or ESWT) is a non-invasive treatment that delivers acoustic pressure waves to injured or dysfunctional tissue. These waves stimulate the body's own healing response, reducing pain and promoting tissue repair.
Shockwave therapy has been used for years to treat conditions affecting muscles, tendons, fascia, and joints. More recently, research has shown promising results for its use in treating erectile dysfunction—and it's now one of the services we offer here at Pelvic Wellness Physical Therapy.
Understanding Erectile Dysfunction
Erectile dysfunction is defined as the inability to get or maintain an erection firm enough for sexual activity.
While age increases the likelihood of ED, it's rarely the only factor. Contributing causes can include:
Diabetes
Heart disease
High blood pressure
Obesity
Certain medications
Anxiety, depression, and chronic stress
Smoking
Excessive alcohol use
At age 40, approximately 40% of men experience some degree of ED. By age 70, that number rises to nearly 70%. But statistics don't tell the whole story—what matters is how it's affecting your quality of life, your confidence, and your relationships.
How Shockwave Therapy Addresses the Root Cause
Here's what makes shockwave therapy different from medications like Viagra® or Cialis®: rather than temporarily managing symptoms, it works by targeting some of the underlying causes of erectile dysfunction.
Research suggests that shockwave therapy may:
Increase blood flow to penile tissue
Stimulate the growth of new blood vessels
Support tissue healing and regeneration
Enhance natural erectile function over time
By improving circulation and promoting tissue repair, shockwave therapy helps create an environment where the body can restore healthy erectile function—not just mask the problem.
A 2022 study by Zanaty and colleagues compared shockwave therapy to tadalafil (Cialis®) and found that both groups experienced significant improvements. However, side effects were reported in only 8% of the shockwave group, compared to 44% in the medication group. For men who want to avoid the side effects of long-term medication use, or who are simply looking for an alternative approach, that's a meaningful difference.
Who Is the Best Candidate for Shockwave Therapy?
The strongest research supports shockwave therapy for men with mild to moderate vasculogenic ED—meaning erectile dysfunction that is primarily caused by reduced blood flow.
You may be a good candidate if:
You have mild to moderate erectile dysfunction
Your ED is primarily related to blood flow or circulatory issues
You currently use, or have previously used, ED medications (Viagra®, Cialis®, Levitra®, or Stendra®) and experienced positive results
Results tend to be less pronounced for men with severe ED or erectile dysfunction following prostate surgery. This is an important distinction—treatment outcomes depend heavily on the underlying cause and severity.
Who May Not Be the Best Fit (Right Now)
Shockwave therapy works by improving circulation and tissue health. If ED is primarily rooted in lifestyle or psychological factors, addressing those first is often the most effective path forward.
Shockwave therapy may not be the best starting point for men under 40 whose symptoms are primarily connected to:
Performance anxiety
Depression or chronic stress
Generalized anxiety disorders
Excessive pornography use
Substance use (alcohol, marijuana, cocaine, steroids)
Smoking
Poor sleep or a sedentary lifestyle
Obesity
This doesn't mean shockwave therapy can never be helpful in these cases—but addressing root causes is usually the most effective first step.
What to Expect: Common Questions
How many treatments are needed? Most patients complete a series of 6 treatment sessions, scheduled weekly or every other week.
How long are appointments? Your initial visit is 45-60 minutes. Follow-up visits are typically 30 minutes.
Does it hurt? No. Most men describe a sensation of pressure from the treatment device, but not pain.
When will I notice results? Everyone responds differently. Some men notice improvements after just 2–3 sessions. Others don't notice significant changes until 2–3 months after their final treatment. This is completely normal. The biological effects of shockwave therapy can continue for 8–12 weeks after treatment ends, as the body continues its healing and regeneration process.
Supporting Your Results: The Bigger Picture
Erectile function doesn't exist in isolation—it reflects overall health. The same factors that affect your heart, your blood vessels, and your nervous system affect your sexual health too.
Alongside shockwave therapy, focusing on the following can make a meaningful difference:
Managing blood pressure and cholesterol
Improving blood sugar control
Achieving a healthy weight
Reducing stress and anxiety
Improving sleep quality
Increasing physical activity
Quitting smoking
Limiting alcohol consumption
Better overall health often leads to better sexual health—and we're here to support the whole picture.
You Don't Have to Just Accept This
Erectile dysfunction is common, but it doesn't have to be something you simply live with. For many men with mild to moderate vasculogenic ED, shockwave therapy offers a non-invasive path to improving sexual function, restoring confidence, and enhancing quality of life—without relying solely on medication.
